submit your information.
Confidentiality Notice: The material on this form is
intended for the use of processing your volunteer application, and potentially
performing background checks for security purposes. It is hereby understood
that the information you have provided is confidential, privileged, and exempt
from disclosure under applicable law.
1. I am aware that this is a volunteer position that requires a fair amount
of athletic ability; running and moving in a large group, and/or pushing and rolling large audio and staging units. Age limits; you must be over the age of 14 for AUDIENCE, and 18 for STAGE/LIGHTING/AUDIO CREW.
2. I have checked the current posted schedule for the group I am volunteering for on this web site, and I am able
to attend the rehearsals for my group in their entirety.
3. I am aware that being a part of the Field Audience or Stage/Lighting/Audio Crew does
not allow me the opportunity to watch the 2007 NFL Kickoff game live inside
the stadium, or access to tickets for the game.
4. I have truthfully completed the form below, and hereby agree to hold harmless
the National Football League, the Indianapolis Colts, White Cherry Entertainment,
all organizers, sponsors, instructors, and others associated with this event
in case of accident or injury. Furthermore, I agree to abide by all written/oral
rules and regulations.
